WHY IS THE CMS QUALIFIED CLINICAL DATA REGISTRY (QCDR) IMPORTANT TO FAMILY PHYSICANS? 
QCDR is an outcomes registry which relies on every Family Physician reporting their outcomes on select patient performance metrics. Improvements in patient populations with chronic illness (such as diabetes, CHD, COPD and asthma) and well-care programs (such as annual vaccine compliance and screening programs) will provide the impetus for improvements in healthcare, reduction in resource utilization, and stabilization of payment rates.


HOW DOES QUALITY MARKERS HELP ME MEET QUALITY REPORTING REQUIREMENTS & AVOID THE NON-REPORTING PENALTIES?
Quality Markers™ extracts meaningful data stored in your EMR. Quality Markers™ analyzes the data on 19 “suites” of care vs national benchmarks of performance.

You can uncover upward or downward trends in your patient population. Improve the average Ha1c for those diabetic patients 65 years or older. Focus on one disease set, i.e. hypertension, for 2017 and report for a month, 90 days, or one year to gain neutral to positive incentive payments. This is the "Pick Your Pace" Quality Reporting Program (QPP) that CMS has chosen for 2017.

 
Control your outcomes with Quality Markers.™ Now that you are able to view your outcomes via easy-to-read graphics and snapshots of your patients, you can put into place clinical and patient education programs to improve outcomes. Search all patients who are diabetic and have not visited your practice for current Ha1c testing. Quality Markers™ tracks and shows you valuable data about your population and patients so you can adjust for optimal patient care.

Upon initiation of Quality Markers™ you will be able to see two years of back data so that you can identify trends in treatment and outcomes. This helps you see where you are and identify where you want to be for 2017.

Unlike EMR’s, Quality Markers can create a list of all patients who are non-compliant with specific measures. This allows you to reach out to select patients who are pulling your quality score down. Once these patients are identified, you can put a treatment plan together to improve their test scores, schedule an annual wellness visit, etc. This is the way to gain incentives, rather than penalties. In 2017, penalties will be up to negative 4%, and incentives range from plus 4%-12%.

CERTIFICATIONS

  • Quality Markers™ is associated with a CMS Qualified Clinical Data Registry (QCDR) which is required for reporting in 2017. The reporting fee is included in the annual subscription price.

  • Quality Markers™ is HIPPA compliant.

  • Quality Markers can be used for credit under Clinical Practice Improvement Activities (CPIA) for 2017 for the "Population Health" category. (See 2017 Payment Ready Resource Kit for more information).

  • Using Quality Markers™ earns Patient-Centered Medical Homes (PCMH) providers 10 auto-credit points towards recognition.
